Check in to your appointment
Before your visit, please make sure you have completed the following at 2-3 days before your appointment:
1) Send us images of your insurance card (front and back), and a photo of yourself
2) Call to give us your payment card information if you have not already done so. We require this on file to cover any copays, deductibles, non-covered amounts, or self-pay fees. We require this in advance, even if you have an in-person appointment, to ensure your costs will be covered and prevent any delays in the appointment.
3) Complete any other intake, history, and consent forms in the portal. For your first appointment, please set aside 20-30 minutes to fill these out.
4) Complete your PHQ-9, GAD-7, Columbia, and any other questionnaires in the portal.
In person
When you arrive, please have a seat in the waiting area by the front desk. Your provider will come to meet you at the waiting area and escort you to their office. If you have a weekend or afterhours appointment, please wait outside or in your car and they will meet you at the front door to let you in to the building and escort you to their office.
Telehealth
Telehealth visits are through Zoom. Please make sure you have downloaded Zoom on your phone or computer. If you need a new link, call our office and we will send you a new one by email or text.
Please keep in mind that we are a small practice. While we will always do our best to assist you, we may not always be able to answer calls and communications immediately.

We are in the Craft Coworking space in the Confluence Construction 2-story building, across from the Red Rocks Medical Center. There is a bus stop next to our building, train lines are within walking distance, and there is plenty of parking.

Our brand new office and company headquarters! Located near the Cherry Creek Reservoir, this office is convenient to Denver, Englewood, Littleton, Highlands Ranch, Greenwood Village, Centennial, and Aurora. Close to bus and train stops, and there is plenty of parking.
